Licensed Practical Nurse (LPN) 7pm-7am

Job Summary:

As a valued member of our healthcare team at The Palms at Sebring, we are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our evening shift from 7pm to 7am. In this rewarding role, you will be responsible for providing high-quality patient care, collaborating with our interdisciplinary team, and contributing to the overall success of our skilled nursing facility.

• Provide direct patient care, including administering medications, wound care, and vital sign monitoring as needed
• Develop and implement individualized care plans in collaboration with the healthcare team
• Conduct regular assessments and evaluations to identify and address patient needs
• Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
• Maintain accurate and up-to-date patient records, including charting and reporting
• Participate in quality improvement initiatives to optimize patient outcomes and facility performance

• Active LPN license in the state of Florida
• 2 years of experience (or equivalent)--we believe in training and supporting our staff to excel in their roles
• High school diploma or equivalent required; additional education or certifications a plus
