Atrium has hired a temporary contractor to serve as a Care Coach 1 assigned to Humana. The role focuses on assessing member needs and guiding families toward resources to improve wellness.
You will monitor progress through assessments, conversations, and care planning, with 75–90% local travel in Highlands County (Sebring/Avon Park area). Mon–Fri, 8 AM–5 PM, TB screening and driver requirements apply.
