Heartland Hospice, part of Gentiva, seeks an experienced Hospice RN / Case Manager to provide in-home nursing care, assess patients, and coordinate services with an interdisciplinary team to ensure compassionate end‑of‑life care.
You will educate families, document care, manage symptoms, and support transitions between care levels. A current RN license and CPR certification are required, with at least one year of RN hospice or related experience preferred.
