Responsibilities

Job Summary

  • Direct and coordinate all nursing care based on established clinical nursing practice standards.
  • Collaborate with other professional disciplines to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes.

Essential Functions

  • Practice nursing as described by the definition: “The diagnosis and treatment of human responses to actual or potential health alterations.” Utilize the nursing process to provide care, including administering treatments and medications, interpreting diagnostic tests, formulating a plan of care, collaborating with other health care providers, and providing education to patients and/or significant others. The successful candidate must pass competencies for this position.

    Involves discretion and independent action within prescribed limits.
  • Collect patient health data.
  • Determine the priority of data collection based on the patient’s immediate condition or needs; collect pertinent data using appropriate assessment techniques; involve the patient, significant others, and health care providers when appropriate; ensure the data collection process is systematic and ongoing; document relevant data in the medical record according to hospital standards.
  • Analyze assessment data to determine diagnoses/patient problems; derive diagnoses/patient problems from the assessment data; validate diagnoses/patient problems with the patient, significant other, and health care providers when possible; document diagnoses/patient problems to facilitate expected outcomes and the plan.
  • Identify and document expected outcomes individualized to the patient; derive outcomes from the diagnoses/patient problems; document outcomes as measurable goals; formulate outcomes with the patient and health care providers when possible; ensure outcomes are realistic given the patient’s capabilities and resources; include a time estimate for attainment; ensure outcomes guide continuity of care and anticipate potential problems and intervene as needed.
  • Develop a plan of care that prescribes interventions to attain expected outcomes; individualize the plan to the patient’s condition or need; develop the plan with the patient, significant others, and health care providers when appropriate; ensure the plan reflects current nursing practice; document the plan; provide continuity of care in the plan.
  • Implement the interventions identified in the plan of care; establish interventions that are aligned with the plan; implement interventions in a safe and appropriate manner; document interventions.
  • Evaluate the patient’s progress toward attainment of outcomes; ensure evaluation is systematic and ongoing; document patient responses to interventions; evaluate the effectiveness of interventions in relation to outcomes; use ongoing assessment data to revise diagnoses/patient problems, outcomes, and the plan of care as needed; document revisions; involve the patient, significant others, and health care providers in the evaluation process when appropriate.
  • Participate in quality-of-care activities, including identifying aspects of care important for quality monitoring; indicators used to monitor quality and effectiveness of nursing care; data collection and analysis of quality data to identify opportunities for improvement; formulate recommendations; implement activities to enhance nursing practice; utilize results to initiate changes in practice.
  • Other duties as assigned.
Qualifications

Education

  • Bachelor’s Degree preferred
  • Graduate of an accredited school of professional nursing

Licenses/Certifications

  • Licensure as a Registered Professional Nurse (RN)

Experience

  • Minimum of one year of hospital clinical experience preferred
