• Assure the safety of each resident when transporting to and from the doctor’s office and/or hospital.
• Accompany each resident on all medical visits and stay until the visit is complete.
• Sign in resident upon entrance and present resident’s documents to office personnel.
• Obtain resident’s documents, new prescriptions, and doctor’s order form; collect appointment cards if a follow‑up is necessary.
• After returning to the facility, report to RCM and be prepared to answer questions concerning the visit.
• Perform other duties as required and assigned.

• Physically able to help ambulatory and non‑ambulatory residents transfer to and from wheelchair and vehicle.
• Possess a valid state driver’s license.
• Qualified as a Certified Nurse Assistant (C.N.A.).
• Maintain a clean driving record.
• Maintain a clean criminal record.
• Hold any other state‑required certifications.
