Responsibilities
  • Assure the safety of each resident when transporting to and from the doctor’s office and/or hospital.
  • Accompany each resident on all medical visits and stay with them until the visit is complete.
  • Sign in residents upon entrance and present their documents to office personnel.
  • Obtain resident’s documents and any new prescriptions before leaving the doctor’s office.
  • Confirm that the doctor has written on the order form.
  • If a follow-up appointment is necessary, obtain appointment cards.
  • Report back to RCM upon returning to the facility and be prepared to answer questions concerning the visit.
  • Perform other duties as required and assigned.
Qualifications
  • Physically able to help ambulatory and non-ambulatory residents transfer to and from wheelchair to vehicle.
  • Possess a valid state driver’s license.
  • Qualified as a Certified Nurse Assistant (C.N.A.).
  • Clean driving record.
  • Clean criminal record.
  • Any other state required certifications.
