Responsible for the provision of primary care medical services in a Patient-Centered Health Home context, with a focus on access to care.

Primary Responsibilities
  • Direct provision of primary care medical services, meeting all established productivity expectations.
  • Conduct examinations and review appropriate laboratory, referral, and imaging results to determine diagnosis.
  • Accurately document visits utilizing appropriate templates and record relevant data to attain a diagnosis.
  • Attain budgeted visits and meet clinical care measures.
  • Lock records in the 72‑hour window.
  • Meet Advanced Practice Provider professional focus and goals.
  • Provide comprehensive, continuous, and coordinated medical care for acute and chronic illness at all ages within a family unit in a community setting.
  • Implement holistic healthcare model focusing on health promotion, prevention, and addressing protective and risk factors to patient wellbeing.
  • Manage chronic diseases and acute illnesses, coordinating care with specialists and utilizing community resources when appropriate.
  • Advocate access to health care with structured evaluation, early intervention, and health promotion.
  • Consult with supervising physician or other team members on cases that need further input.
  • Work within and promote the Patient-Centered Health Home model of care.
  • Provide excellent customer service, behaving courteously when interacting with patients, family members, support staff, and Network staff.
  • Maintain awareness of wait times during daily workflow.
  • Provide patients with educational materials and resources appropriate to their health literacy.
  • Ensure patient status as measured by relevant clinical quality measures.
  • Promote and participate in daily huddles.
  • Incorporate participation of ancillary health team members in managing care of patients/families.
  • Set and monitor self‑management plans.
  • Assure clinical procedures and maintain up‑to‑date clinical protocols.
  • Complete chart and peer review responsibilities promptly.
  • Assist the patient care team with performance improvement efforts, staff education, and FHCN promotion.
  • Assist with performance evaluations and quality improvement.
  • Where relevant, participate in rotations of medical, physician assistant students, and dental residents.
  • Participate in Network Health Fairs and other community events.
  • Participate in site and provider meetings in a meaningful way.
  • Meet Network and credentialing expectations for licensure and employment.
  • Adhere to Attendance and Absenteeism Policy; recognize regular attendance is essential.
  • Present to and work at any FHCN location, at beginning or during shifts, based on business needs.
  • Perform other duties as assigned.
Primary Attributes & Qualifications Professional & Technical Knowledge
  • Possesses specific advanced knowledge and skills, including written and verbal communication, computational, computer, technical skills, and mathematical knowledge.
  • Completion of an accredited Nurse Practitioner or Physician Assistant training program with a minimum cumulative GPA of 2.5.
  • Successful attainment and maintenance of a valid, active professional license required for the role, issued by the appropriate governing or regulatory body.
Advanced Training Requirements

Performance of the duties requires the equivalent of extensive formal training as a Nurse Practitioner or Physician Assistant, including understanding the application of the theory and practices of the profession as part of a Master’s degree or other applicable extended training program.

Technical Skills
  • Ability to create highly complex documents in Microsoft Word, including linking multiple files and embedding objects linked to other documents.
  • Ability to use advanced functions of Microsoft Excel, such as creating and managing databases, standardized reports, and linking multiple worksheets and workbooks.
  • Ability to develop sophisticated presentations in Microsoft PowerPoint, including embedded objects, transitions, and other elements.
Communication Skills
  • Effectively communicate professional opinions and extrapolations of information, synthesizing and analyzing data.
