Care Lync seeks a Supports Coordinator to provide service coordination for children birth to age 6 and their families. Under supervision, you will help families access services, understand rights, and receive coordinated early intervention supports that promote child development and family capacity.
You will coordinate evaluations, develop IFSPs, monitor service delivery, and collaborate with families, providers, and agencies.
