Position Responsibilities
  • Adheres to and conveys philosophy of supporting dignity, privacy, independence, choice, individuality, and a home-like environment for Resident.
  • Receives and gives report from the previous shift.
  • Reviews assignments with Designated Supervisor.
  • Reads the staff communication records and maintains familiarity with Resident records as required.
  • Assists with instrumental activities of daily living, ADL’s, environmental orientation, assistance with other care while encouraging self‑care and independence.
  • Provides services as indicated on Resident service plans, including but not limited to bathing assistance, toileting assistance, transfer, and mobility assistance.
  • Demonstrates knowledge of and follows infection control procedures.
  • Maintains clean, neat, comfortable, safe environment for Residents, staff, and visitors.
  • Observes changes in Resident status, needs, or preferences and communicates them appropriately.
  • Adheres to all established policies and procedures.
  • Records Resident information as required.
  • Demonstrates knowledge of and adheres to procedures for fire, life safety, disaster, security, work, safety, and other emergency situations.
  • Notifies nurse, and/or designated agent(s) of Resident status/change as appropriate.
Training and Experience

Completion of Certified Nursing Assistant program with current license. 18–24 months experience in a similar position, CPR/First Aid certified preferred.

Education

Attendance at mandatory in‑service training. Maintain CPR/First Aid Certification. Meet in‑service training hours required for job classification and position.
