Sequim, WA 98382 — Position Type:
Full Time — Salary Range: $40.00–$47.00 hourly.
The Night Shift LVN implements the established plan of care for each assigned group of residents. Responsibilities include total care for chronically ill and technologically dependent residents, administration of medications, performance of treatments, provision of resident/family education, and maintenance of a record of the care provided. Under the direct supervision of the RN, assists with planning, coordination, and provision of individualized resident care according to facility policies and procedures.

- Perform resident care duties to meet physical, psycho‑social, and spiritual needs of chronically ill and technologically dependent residents, incorporating cultural and ethnic factors.
- Complete initial and ongoing assessments, gathering data in a timely manner and incorporating functional/developmental age factors.
- Differentiate between normal and abnormal clinical findings and intervene per clinical standards and physician orders.
- Collaborate with residents/families and the interdisciplinary team to develop individualized care plans.
- Demonstrate sound clinical judgment in implementing and evaluating nursing aspects of the care plan.
- Accurately monitor and report resident condition changes to RN, attending physician, families, team members, and Director of Nursing or ADON.
- Follow through on care services needed to meet individualized needs.
- Administer medications proficiently, including pain management.
- Provide IV care if certified, within scope of practice and facility policy.
- Administer treatments under physician direction (e.g., wound packing, irrigation, catheter care).
- Provide and manage care of technologically dependent residents:
- Ventilator dependency
- Ostomies (tracheostomy, ileostomy, urostomy, gastrostomy, etc.)
- Enteral feeding tubes
- Tracheostomy care
- Suctioning
- Participate in resident/family education needs, provide individualized teaching considering functional/developmental age, and document effectiveness.
- Demonstrate knowledge of age‑specific care, safety measures, normal values, skin integrity, behavior, motor skills, and risk behaviors; communicate effectively.
- Identify signs of abuse/neglect and report/document per policy.
- Respect residents' rights, dignity, and individuality.
- Maintain confidentiality of resident and facility information.
- Show honesty and integrity at all times.
- Provide care in emergency situations and follow procedures.
- Maintain knowledge and implement care activities to promote health.
- Ensure residents' privacy and dignity during care.
- Communicate orally and in writing with residents, families, physicians, and coworkers.
- Document thoroughly and accurately in prescribed style.
- Perform other duties as assigned.
- Move intermittently throughout the workday.
- Continuous bending, squatting, twisting, and reaching.
- Visual and auditory abilities adequate for role (including prosthetic use if needed).
- Meet general health requirements (annual TB screening, physical exam).
- Stand and walk for extended periods.
- Lift and carry up to 25 lbs.
- Infrequent lifting of weight up to 100 lbs with assistance.
- Frequently push and pull carts; transport residents.
- Cope with mental and emotional stress of the position.
Work inside; normal temperatures; some noise; occasional fumes/odors; chemical exposure; potential exposure to blood borne pathogens.
Qualifications/Requirements- Education: LVN graduate from an accredited School of Nursing.
- License: Current, valid LVN licensure in the state; current BCLS certification.
- Work Experience: Minimum 6 months prior LVN experience in past 2 years in a general acute‑care hospital, or equivalent competency for sub‑acute residents.
- Language: Ability to read, analyze, and interpret common scientific and technical information; communicate verbally in English.
- Mathematical: Ability to perform basic arithmetic in units of weight and volume.
- Communication: Exceptional communication and customer‑service skills; empathy; ability to communicate with patients, families, staff, and external resources.
