LTSS Service Coordinator – RN Clinician (RN Case Manager)

We are hiring statewide across Ohio. Must reside in Ohio. This field‑based role allows travel to client sites; office attendance for meetings or training as needed.

• Perform telephonic and face‑to‑face functional assessments to identify, evaluate, coordinate, and manage members’ needs.
• Identify members for high risk complications and coordinate care with the member and health‑care team.
• Manage members with chronic illnesses, co‑morbidities or disabilities; ensure cost‑effective utilization of health benefits.
• Obtain thorough member history to develop individual care plan and set short & long‑term goals.
• Develop and revise care plans, authorize referrals, and facilitate authorizations as appropriate.
• Interface with Medical Directors, Physician Advisors, interdisciplinary teams to develop person‑centered care plans.
• Assist with problem‑solving with providers, claims, or service issues.

• High school diploma or GED equivalent plus minimum 3 years experience in case management for individuals with chronic illnesses, comorbidities, or disabilities.
• Current, active, valid and unrestricted RN license in Ohio.
• BA/BS in Health/Nursing preferred.
• Strong preference for case management experience with older adults or individuals with disabilities.

Location:

Columbus, OH (field‑based) – Candidates must reside within Ohio; travel to client sites as required.
