PositionSummary
As an LPN at Freedom Senior Living, you will play a vital role in ensuring the health, safety, and comfort of our residents. Working closely with residents, families, healthcare providers, and interdisciplinary team members, you’ll provide direct nursing care in accordance with physician orders, facility policies, and state regulations. Key Responsibilities- Administer medications and treatments as prescribed by physicians.
- Monitor residents’ physical and emotional conditions, reporting significant changes to healthcare providers.
- Conduct nursing assessments and document findings accurately and promptly.
- Assist in developing, implementing, and updating individualized care plans for residents.
- Provide specialized care such as wound care and catheter care within the LPN scope of practice.
- Supervise and guide Certified Nursing Assistants (CNAs) and other direct care staff.
- Maintain accurate documentation of resident care, medication administration, and incidents.
- Respond promptly to emergencies and initiate appropriate interventions.
- Communicate effectively with residents, families, physicians, and other healthcare professionals.
- Uphold infection control standards and safety protocols.
- Participate in shift reports, staff meetings, and quality improvement initiatives.
- Assist with admissions, transfers, and discharges as needed.
- Ensure compliance with federal, state, and facility regulations.
Education and Licensure: - Graduate of an accredited Practical Nursing program.
- Current, unrestricted Licensed Practical Nurse (LPN) license in the state of Tennessee.
- CPR/BLS certification preferred or required.
Experience: - While no prior experience is required, experience in assisted living, long‑term care, geriatrics, or skilled nursing is preferred.
- Familiarity with medication administration and electronic health records is a plus.
Skills and Abilities: - Strong knowledge of nursing principles, medication administration, and resident care practices.
- Excellent assessment, observation, and communication skills.
- Ability to recognize and respond to changes in resident conditions.
- Strong organizational and time‑management skills.
- Ability to work independently and as part of a multidisciplinary team.
- Commitment to maintaining confidentiality and professional boundaries.
Physical Requirements: - Ability to stand, walk, bend, stoop, and lift up to 50 pounds with or without reasonable accommodation.
- Ability to assist residents with mobility and transfers.
- Quick response capability in emergency situations.
Work Environment: - Assisted Living/Residential Care setting.
- Exposure to infectious diseases and bodily fluids.
- May require weekends, holidays, overtime, and on‑call coverage as needed.
