Social Worker Position

This role provides essential psychosocial support to patients and families navigating chronic, acute, or terminal illness. Through counseling, education, care coordination, and connection to community resources, the position helps reduce barriers to care, strengthen coping and resilience, and promote equitable, patient-centered health outcomes across the continuum of care.

Key Position Details:

• .6 FTE (48 hours per two-week pay period)
• 8-hour day shift
• 1 weekend in every 6
• MNA union position

Job Description:

Provides patients and families with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services include advising family caregivers, providing patient education and counseling, and making referrals for other services. May also provide care and case management or interventions designed to promote health, prevent disease, and address barriers to access to healthcare.

Principle Responsibilities

• Delivers professional and thorough social work services, including psychosocial assessment and intervention planning.
• Coordinates complex transition plans with patients, families, health care team and community providers.
• Advocates for patients and families by supporting patient rights and accessing protective services.
• May participate in care system process that prevent readmissions.
• May collaborate with health care team to promote appropriate length of stay.
• Other duties as assigned.

Required Qualifications

• Bachelor's degree in Social Work
• 0 to 2 years of experience in a health care setting

Preferred Qualifications

• Master's degree in Social Work
• 0 to 2 years of experience in an acute care setting

Licenses/Certifications

• Licensed Social Worker - MN Board of Social Work required

Physical Demands

• Sedentary:
• Lifting weight up to 10 lbs. occasionally, negligible weight frequently

Pay Range

Pay Range: $37.16 to $46.45 per hour

The pay described reflects the base hourly pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable). Shift, weekend and/or other differentials may be available to increase your pay rate for certain shifts or work.
