Overview
Local home health agency is seeking an experienced LPN to provide one-on-one services to patients in their homes.
Responsibilities- Provide medical services to patients.
- Complete in-home assessments and intakes.
- Assist with admissions.
- Complete 90-day follow-up care.
- Ensure the patient care plan is being followed.
- Complete required notes and documentation.
- Educate patients and families on care, treatments and answer questions.
- Must be a licensed LPN.
- Experience in a home health care setting is required.
