Local home health agency is seeking an experienced LPN to provide one-on-one services to patients in their homes.
- Duties would include providing medical services to patients.
- Completing in home assessments and intakes.
- Assist with admissions.
- Complete 90 day follow-up care.
- Ensure the patient care plan is being followed.
- Complete required notes and documentation.
- Educate patients and families on care, treatments and answer questions.
Employer has full and part time openings.
