Scope of Work

Provides therapy to patients based on patient need and medical direction. Provides patient/caregiver education and arranges for adaptive equipment as needed.

Qualifications
  • Bachelor's Degree or equivalent from an accredited physical therapy program
  • Master's Degree (preferred)
  • 6 months of relevant experience (preferred)
  • Licensed Physical Therapist – State of Michigan
  • Upon Hire required LIC-Driver's License – State of Michigan
  • Upon Hire required CRT-Basic Life Support (BLS) – American Heart Association (90 days required)
  • Or CRT-Basic Life Support (BLS) – American Red Cross (90 days required)

SITE – Care Center Wyoming, 6105 Wilson – Wyoming Department (Rehab Wyoming Care Ctr – Grand Rapids Hosp)

Employment Type:

Part time.

Shift: Day (United States of America). Weekly Scheduled

Hours:

4. Hours of Work: 8:00am – 6:00pm. Days Worked Monday to Friday. Weekend Frequency: N/A.
