Frontpoint Health seeks a PRN Physical Therapist (PT) to deliver home health services to patients in their own homes across designated territories. You will assess conditions, develop and implement treatment plans, and monitor progress to adjust care as needed.
You will collaborate with other health professionals to ensure comprehensive care, maintain accurate records, and educate patients and families about therapy goals. A valid state license and at least one year in home health are required.
