- Conduct in-clinic, face-to-face visits with dialysis patients and family members to understand daily challenges and support needs
- Provide compassionate education encouraging openness to in-home care support
- Establish and maintain customer relationships through exceptional service and care coordination
- Meet or exceed monthly admission targets and KPIs
- Guide customers through referrals, service recommendations, and admissions
- Engage clients and families, assess satisfaction, and resolve service issues to support retention
- Enter and update customer information and engagement activities in the BOLT CRM system
- Identify cross-sell and upsell opportunities based on customer needs
- Coordinate services through Home Care Home Base and connect with the care team
- Participate in ongoing training on product knowledge, sales techniques, and industry trends
- Maintain patient and staff privacy and confidentiality under HIPAA Privacy Final Rule
- Report to the Manager Business Operations
- Travel 90% to local service locations in an assigned regional area
- Graduate of an accredited LVN/LPN program
- Current state license as LPN/LVN
- Minimum of one year of experience in home care or the healthcare industry, preferably as a nephrology nurse
- Discharge planning experience and/or healthcare sales experience preferred
- Previous experience in home care, care coordination, case management, or patient education
- Familiarity with Chronic Kidney Disease stages and dialysis workflows
- Sales, outreach, or client-engagement experience, preferably in healthcare
- In-depth knowledge of Medicare/Medicaid, MCO, MLTC, other payer types, and home care benefits, rules, and regulations across multiple states
- Ability to use computer and data-entry systems, Microsoft Office, and database software
- Strong written and verbal communication skills
- Bilingual preferred
- Dependable vehicle
- Valid driver's license
- Current auto insurance in accordance with state law
- Ability to sit, stand, bend, lift, and move intermittently
- Ability to lift at least 25 lbs.
- Ability to travel 90% to local service locations
Demonstrates expertise in patient education, care coordination, and relationship management within the healthcare sector, particularly in home care and nephrology. Proficient in navigating healthcare regulations and utilizing CRM systems to enhance service delivery and client engagement.
