Physical Therapist evaluates, develops, and implements individualized treatment plans designed to restore function, improve mobility, reduce pain, and enhance quality of life. This role delivers skilled physical therapy services in accordance with physician orders, professional practice standards, and Advent Health policies while promoting patient safety, clinical excellence, and positive outcomes.

Key Responsibilities
  • Evaluate patients using evidence-based assessment techniques to determine functional limitations and rehabilitation needs.
  • Perform comprehensive re-evaluations to assess patient progress and modify treatment plans as appropriate.
  • Develop, implement, and adjust individualized treatment plans, including measurable goals, therapeutic interventions, and treatment frequency.
  • Provide skilled physical therapy interventions to improve mobility, strength, balance, coordination, endurance, and functional independence.
  • Educate patients, families, and caregivers regarding treatment plans, home exercise programs, injury prevention, and safe mobility techniques.
  • Document patient evaluations, treatments, progress notes, and discharge summaries accurately, objectively, and within established time frames.
  • Collaborate with physicians, nurses, case managers, and interdisciplinary team members to coordinate comprehensive patient care.
  • Maintain current clinical knowledge by utilizing evidence-based practice, participating in professional development, and supporting continuous performance improvement initiatives.
  • Serve as a Clinical Instructor and mentor students when applicable.
  • Promote patient and employee safety by following department policies, infection prevention practices, and regulatory requirements.
  • Represent Advent Health through participation in community outreach, educational programs, and wellness initiatives as opportunities arise.
  • Maintain compliance with all hospital policies, accreditation standards, and applicable local, state, and federal regulations.
Knowledge, Skills & Abilities
  • Knowledge of physical therapy principles, evaluation techniques, and evidence-based treatment practices
  • Ability to develop and implement individualized treatment plans
  • Strong clinical reasoning and problem-solving skills
  • Excellent patient assessment and documentation skills
  • Ability to communicate effectively with patients, families, physicians, nurses, administrators, and interdisciplinary healthcare teams
  • Excellent verbal, written, and electronic communication skills
  • Basic computer proficiency, including Microsoft Office, Outlook, Windows, and Electronic Medical Record (EMR) systems
  • Commitment to patient safety, quality improvement, and professional development
  • Ability to build positive relationships with patients and colleagues from diverse backgrounds
  • Ability to read, write, and communicate effectively in conversational English
