** The*
* ** Behavioral Health Social Worker/Therapist*
* ** provides comprehensive behavioral health services to patients experiencing mental health and behavioral health concerns. This role conducts psychosocial and risk assessments, develops individualized treatment plans, provides individual, family, and group therapy, and collaborates with physicians and the interdisciplinary care team to support positive patient outcomes. The Behavioral Health Social Worker/Therapist also coordinates discharge planning, facilitates referrals, maintains accurate clinical documentation, and ensures continuity of care while delivering compassionate, patient-centered services.*
* ** Key Responsibilities*
* + Conduct comprehensive  
** behavioral health assessments** , including psychosocial evaluations and risk assessments.

+ Develop individualized  
** treatment plans
** with measurable therapeutic goals based on patient needs and clinical findings.

+ Provide  
** individual, family, and group therapy
** utilizing evidence-based therapeutic interventions.

+ Evaluate patient progress and modify treatment plans based on ongoing clinical assessments.

+ Document assessments, treatment interventions, patient progress, and outcomes accurately while maintaining patient confidentiality.

+ Collaborate with  
** physicians, nurses, case managers, and interdisciplinary treatment team members
** to develop and coordinate patient care plans.

+ Participate in  
** multidisciplinary treatment team meetings
** by providing clinical recommendations and patient progress updates.

+ Develop comprehensive  
** discharge plans
** in collaboration with patients, families, and the interdisciplinary team, ensuring appropriate outpatient appointments and community resources are arranged prior to discharge.

+ Coordinate referrals to appropriate internal services and community behavioral health resources to promote continuity of care.

+ Identify and appropriately intervene in cases involving  
** suspected abuse or neglect** , demonstrating sensitivity to age-specific and patient-specific concerns.
