Frontpoint Health is seeking a full-time Registered Nurse (RN) for home health care to manage and coordinate patient care in their place of residence. Candidates must hold a current Texas State license as an RN and possess at least 2 years of home health experience.
This role includes performing comprehensive patient assessments, providing skilled nursing care, and ensuring confidentiality under HIPAA laws. Reliable transportation and CPR certification are also required. The RN will work closely with an interdisciplinary team to ensure high-quality care for patients.
