In-Home Nurse Practitioner Part Time - Pittsburg, KS

Overview

Signify Health, part of CVS Health®, seeks a Part‑time Nurse Practitioner to conduct In‑Home Health Evaluations. You will visit patients in their homes, provide medication and medical history review, perform a physical exam, and administer selected diagnostic tests.

The visit lasts about 45 minutes, allowing time for a one‑on‑one conversation, health questions, and if ordered by the health plan, additional diagnostic tests (e.g., diabetic eye exams, spirometry).

You will use an iPad and an intuitive clinical workflow. Prescription, lab ordering, or treatment regimens are not required for this role.

• Build trusted relationships with patients and coordinate care recommendations.
• Conduct in‑home evaluations and perform physical assessments.
• Provide diagnostic and preventive services as indicated (spirometry, diabetic eye exam, etc.).
• Travel locally and within licensed states as needed.
• Conduct virtual visits as required.

Your primary workplace will be the private residence of health‑plan members. You must be able to perform duties safely in environments that may contain pets, secondhand smoke, or allergens.

Pay Range: $37.66 – $81.11 per hour (or equivalent annual salary). The actual rate depends on experience, education, geography and other factors.
