Job Summary:

The PACE Center Coordinator – LPN supports the coordination of daily PACE center operations and clinical workflows to promote safe, efficient, and participant-centered care. This role serves as an on-site clinical support resource and works collaboratively with the RN Care Coordinator, providers, and interdisciplinary team (IDT) to communicate participant needs, coordinate services, and facilitate timely follow-through on care plans.

The Center Coordinator – LPN does not have direct supervisory responsibility and functions in a supportive and coordinating capacity. The role assists with daily center activities, participant flow, clinical information gathering, and communication between center and clinical teams.

Duties/Responsibilities:

Coordinate daily center activities to support efficient participant flow and service delivery.

Conduct routine rounds to identify participant safety, engagement, and environmental needs.

Coordinate participant check-in and assist with maintaining established center processes.

Help maintain a clean, safe, organized, and well-stocked center in collaboration with Central Supply.

Receive and communicate participant concerns to the appropriate team member for follow-up.

Serve as an on-site clinical support resource for participant needs.

Gather and document participant symptoms, vital signs, and changes in condition.

Communicate changes in participant condition to RN Care Coordinators and providers for assessment and direction.

Escalate urgent or emergent concerns according to established protocols.

Reinforce care plans, provider instructions, and follow-up needs as directed by the IDT.Assist with coordination of care transitions following hospital, emergency department, or skilled nursing stays.

Coordinate home care scheduling needs and communicate changes in participant coverage.

Collaborate with Home Care Coordinators regarding scheduling needs, gaps, call-outs, and participant-specific changes.

Communicate schedule updates and service changes to appropriate staff.

Assist with coordinating daily adjustments based on participant needs and staffing availability.

Participate in IDT meetings and communicate relevant participant updates, risks, and needs.

Coordinate communication among nursing, providers, therapy, social services, intake, and other members of the IDT.Support implementation and follow-through of participant care plans under the direction of clinical leadership.

Serve as a communication link between center operations and clinical teams.

Assist with onboarding, training, and competency activities as assigned.

Provide support and coverage within the center during staffing shortages or changing participant needs.

Support accurate, timely, and complete documentation in the electronic medical record.

Assist with follow-through of provider orders and clinical recommendations as directed.

Participate in chart reviews, audits, and quality checks as assigned.

Support compliance with CMS, state, PACE, and organizational requirements.

Maintain HIPAA compliance and participant confidentiality.

Follow and reinforce established infection control, safety, and emergency preparedness procedures.

Identify and communicate operational, clinical, safety, or participant concerns to the appropriate leadership or clinical team member.

Participate in quality improvement activities and organizational initiatives.

Participate in committees and other PACE-related activities as assigned.

Perform additional duties as assigned.
