Care Manager

About This Role

The Care Manager develops, assesses, and facilitates complex care management activities primarily for members with mental and behavioral health needs, ensuring high-quality, cost-effective healthcare outcomes.

• Pay: $56,200 - $101,000/year
• Location: Remote, OK
• Residency: Must reside in Oklahoma, Canadian, Cleveland, or Logan County, Oklahoma
• Schedule: Full-time, Day shift

• Evaluate member needs via phone or in-home visits and recommend/facilitate care plans.
• Perform telephonic, digital, home, and/or other site visits outreach to assess member needs.
• Develop ongoing care plans for members with high acuity and identify necessary providers and resources.
• Coordinate between the member, family/caregivers, community resources, and care provider team.
• Monitor care plans and member status for changes in treatment and provide recommendations.
• Facilitate care coordination and collaborate with providers or specialists for timely access to care.
• Collect, document, and maintain member information and care management activities.
• Provide education to members and families on procedures, treatment options, and healthcare benefits.
• Provide feedback to leadership on opportunities to improve care and quality delivery.
• Perform other duties as assigned and comply with all policies and standards.

• Education:
  • Master's degree in Behavioral Health or Social Work, or a Degree from an Accredited School of Nursing
• Experience:
  • 2 – 4 years of related experience

  • Licensed Master’s Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC), OR
  • RN (Registered Nurse) based on state contract requirements with BH experience
