- Ensure that all activities performed align with the vision of Elara Caring’s board of directors, executive team, and the leadership of the Hospice team.
- Assess hospice care patients and families to identify the physical, psychosocial, and environmental needs of patients as evidenced by documentation, clinical records, interdisciplinary team reports, after‑hours reports, and on‑site evaluations.
- Assess patient needs and obtain data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status.
- Communicate significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the clinical supervisor, physician, facility, and/or other personnel involved with patient care.
- Counsel, instruct, and include the patient, facility, and family in following the interdisciplinary plan of care.
- Teach hospice philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted‑living facilities, etc.).
- Provide appropriate support at the time of death and perform bereavement assessment.
- Graduate of an accredited certificate, diploma, associate, or baccalaureate school of nursing.
- Current state license as a registered nurse (RN).
- One year of experience in a clinical care setting.
- Experience in a hospice or home health environment is preferred.
- 50% travel required.
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
- Valid driver’s license, insurance, and reliable transportation to perform job tasks.
You will report to the Clinical Team Manager.
