The UCAT Assessment Registered Nurse (RN) serves as a key member of the PACE enrollment and clinical team, conducting comprehensive pre-enrollment nursing assessments in participants' homes to determine clinical eligibility for the PACE program. This position plays an integral role in identifying and engaging potential PACE participants, educating prospective participants and families, and collaborating with the interdisciplinary team to support informed enrollment decisions.

The UCAT Assessment RN ensures accurate assessment of participants' medical, functional, cognitive, psychosocial, and environmental needs while helping maintain participant safety and quality of care throughout the enrollment process.

In addition to admissions assessment responsibilities, the UCAT Assessment RN provides clinical nursing support to the PACE program as needed, including participation in the on-call rotation, collaboration with the interdisciplinary team, and coverage for RN Care Coordinators during absences or periods of increased operational need.

The RN ensures compliance with federal, state, and CMS PACE regulations while delivering participant-centered, high-quality nursing care throughout the participant lifecycle - from intake through ongoing enrollment.

• Current Registered Nurse (RN) license in good standing (state-specific).
• Current CPR Certification.
• Valid Driver's License (for field-based work).
• ACLS and UCAT certifications preferred (as applicable).
• Experience in geriatric, home health, or PACE preferred.
• Prior experience with in-home assessments and care coordination strongly preferred.

• Ability to travel regularly by vehicle.
• Lift up to 50 lbs (occasional).
• Walk up to 0.25 miles (frequent/occasional).
• Stand, sit, bend, squat, kneel, and reach as required.
• Push/pull up to 75 lbs of force (occasional).
• Full range of motion and balance for patient care and field work.
