We are seeking an experienced Nurse Practitioner or Physician Assistant to join a collaborative hospital medicine team providing high-quality care in a post-acute hospital setting serving the Sheboygan, Wisconsin area.

Position Overview

The Advanced Practice Provider will work closely with physician leaders to provide inpatient care for medically complex patients in a post-acute hospital setting.

You'll help manage patients with higher-acuity medical conditions while collaborating with physicians, hospitalists, nursing staff, and clinical leadership to improve patient outcomes and maintain exceptional quality metrics.

  • Tracheostomy and ventilator management
  • Complex wound care
  • Multiple chronic medical conditions
  • Acute changes in patient status
  • Post-acute and medically complex inpatient populations
Responsibilities
  • Provide comprehensive inpatient medical management
  • Evaluate and treat medically complex patients
  • Manage acute clinical changes and interventions
  • Collaborate daily with supervising physicians and hospitalists
  • Participate in weekend rotations as part of the 7-on/7-off schedule
  • Coordinate care with interdisciplinary clinical teams
  • Maintain timely and accurate documentation
  • Support quality initiatives and evidence-based clinical protocols
  • Assist in achieving patient outcome and readmission goals
Qualifications Required
  • Licensed Nurse Practitioner or Physician Assistant (or ability to obtain Wisconsin licensure)
  • Minimum of 3–5 years of acute inpatient hospital experience
  • Ability to work independently while collaborating with physicians
  • Strong clinical decision-making skills
  • Excellent communication and organizational abilities
Preferred
  • Experience managing medically complex adult patients
  • Post-acute, hospital medicine, critical care, or internal medicine experience
  • Comfort managing ventilator, tracheostomy, and complex wound patients
Schedule
  • Full-Time
  • 7-On/7-Off
  • Approximately 10 hours onsite each day
  • Shared call coverage for the remainder of the shift
  • Weekend rotations included
