Advocate Health Care - 3400 Union Avenue - Responsibilities:
Assist RN with daily patient care including bathing, oral hygiene, feeding, activity orders, and ambulation;
Measure and document blood pressures, temperature, pulse, respiration, intake and output, daily weights, and other patient data;
Document accurately in Electronic Medical Record;
Communicate observations and patient/family needs to RN;
Assist with admission, transfer, and discharge processes including room setup and patient belongings
