RN Care Manager- La Crosse​/Sparta

About This Role

As a RN Care Manager, you will support members enrolled in the Wisconsin Family Care Program by providing essential care coordination, support, and education, all while enjoying a true work-life balance with no night or weekend shifts.

• Schedule:
  
  Full-time, Monday through Friday, 8:00 am - 4:30 pm CST
• Travel:
  Up to 40%
• Pay: $71,100 - $97,800 per year
• Sign-On Bonus:
  Eligible for a bonus incentive plan

• Assess members to develop a comprehensive Member Care Plan (MCP).
• Conduct regular health assessments.
• Coordinate with acute and primary care providers.
• Educate members on disease processes, nutrition, exercise, and lifestyle modifications.
• Document in member files within required DHS contract time frames.
• Participate in member monthly phone contacts and travel for in-person visits.
• Complete record reviews for reassigned members.
• Provide support regarding record review and remediation.
• Ensure newly assigned RNs and their Care Coach partners are informed of follow-up needs.
• Accept additional complex member assignments.

• Education:
  
  Associate degree in nursing required
• Experience:
  
  3 years Care Management experience preferred
• Licenses &
  
  Certifications:
  • Licensed Registered Nurse in the state of Wisconsin (in good standing) required
  • Valid state driver's license and proof of personal vehicle liability insurance required
