Intensive Support Services (ISS) provides mentoring, individual skill development, and support and resources to Sheboygan County youth and their families. The ISS Youth Counselor meets with youth and families to work towards their goals and fulfilling their court ordered obligations. This program can also include electronic monitoring, as directed by Sheboygan County, enabling youth to safely remain in the community. This position meets with clients Monday through Friday, primarily in the community, assisting with transportation, as needed.
Responsibilities- Review referrals and respond to the referring Social Worker to discuss information to guide service planning and approaches
- Schedule and complete intakes to initiate services with youth and families
- Develop individualized Service Plans and review and revise plans as needed
- Consult with Program Director to tailor evidence-based services to meet the needs of the youth and their family
- Provide mentoring, individual skills building, coordination and/or referral to prosocial activities, and assistance in obtaining employment
- Meet with and or coordinate daily contacts Monday through Friday; collaborate with weekend and holiday providers
- Support family communication and connect youth and families to community resources
- Enter youth information into electronic monitoring software, respond to alerts, and retrieve GPS monitoring equipment
- Administer Personal Growth & Mindset Survey at intake and exit
- Complete documentation in a timely manner, including regular communication with Social Worker, and monthly progress reports
- Accurately document and track required data in accordance with established program policies and procedures
- Complete Discharge Summary outlining the youth’s progress towards achieving goals in the Service Plan, improvement in functioning in areas such as employment, school, substance use, self-esteem, and mental health symptoms, and recommendations for follow-up care
- Participate in meetings with school staff, Social Workers, and other providers as needed
- Other duties as assigned, including help with coverage at Innov8 Youth Center
- Ability to maintain and promote positive working relationships with others
- Understanding of and experience providing services using strength-based approach, Motivational Interviewing, and Trauma Responsive Care
- Ability to collaborate with clients in various settings, including their homes, the community, and our office, to provide comprehensive support and services
- Exercise good decision making in the field and strong ability in de-escalating situations
- Effective use of time and resources to help maintain quality services
- Ability to work well with individuals from diverse economic, racial, and ethnic backgrounds and LGBTQ+ identities
- Maintain confidentiality and boundaries
- Knowledge of community resources
- High level of skill in organization, attention to detail and accuracy as well as ability to handle multiple tasks at one time
- Strong and oral and written communication
- Proficient computer skills required, including use of Microsoft Office products and ability to learn
- Bachelor’s Degree in Social Work or related field
- At least two (2) years of experience working with youth with mental health &/or substance use history, preferred
- Pre-employment background check is required
- Valid driver’s license and automobile with insurance
