Responsibilities
  • Provide case management services to youth and families involved with the Youth Justice system to promote public safety, accountability, rehabilitation, and positive youth development, in accordance with Wisconsin Statutes, court orders, and Department policy.
  • Conduct comprehensive assessments of youth and families through interviews, school visits, home visits, collateral contacts, and review of records to evaluate strengths, needs, risks, and protective factors.
  • Review and process youth justice referrals received from law enforcement; conduct intake inquiries with youth and families; apply approved assessment tools to inform recommendations; and participate in case staffing and decision-making regarding appropriate disposition or court action.
  • Develop, implement, and monitor individualized case plans in collaboration with youth, families, and service providers, including identifying goals, required actions, timelines, and appropriate interventions.
  • Provide ongoing case management and supervision services for youth involved through informal agreements or court orders, including monitoring compliance with conditions, responding to violations, and initiating appropriate court action when necessary.
  • Respond to crisis situations involving youth safety, community safety, or court violations; coordinate with law enforcement, detention facilities, schools, and other partners; and provide consultation and authorization regarding custody or detention decisions as required.
  • Prepare court-related documentation, reports, and recommendations; attend hearings; and provide written and oral testimony as required throughout the life of a case.
  • Maintain accurate, timely, and complete case documentation in the State of Wisconsin’s automated child welfare and youth justice systems, including case notes, assessments, service plans, reviews, referrals, and closure documentation.
  • Coordinate and participate in multidisciplinary team meetings, service staffings, and consultations with schools, treatment providers, alternate care providers, community agencies, and internal County partners.
  • Provide direct support to youth and families, including skill building in areas such as emotional regulation, decision-making, communication, and problem-solving, and connect families to appropriate community services and resources.
  • Monitor and review youth progress, service engagement, restitution and community service obligations, and compliance with court or informal agreements; adjust case plans as needed based on assessment and outcomes.
  • Conduct required reassessments, reviews, and evaluations at designated intervals and at case closure, using approved assessment tools and documentation standards.
  • Maintain regular consultation with supervisory staff regarding case direction, legal requirements, and complex decision-making.
  • Participate in unit, division, department, and County meetings; interdisciplinary work groups; and required training, professional development, and licensure maintenance activities.
  • Apply trauma-informed principles, cultural humility, and ethical standards in all interactions with youth, families, colleagues, and community partners.
  • Perform administrative and program support duties necessary to carry out assigned responsibilities, including completion of required reports, statistics, and documentation.
  • Perform other related duties as assigned to support the mission, programs, and operations of the Health and Human Services Department.
Qualifications
  • A minimum of a bachelor’s degree from an accredited college or university in social work or a closely related field.
