Case Manager

Humana is seeking a Case Manager in Sheboygan to support members with complex medical and social needs. In this role, you'll complete holistic assessments, build individualized care plans, and coordinate services across providers, community resources, and benefits programs. You will educate members and families, address barriers to care, and track progress using data and documentation tools.
