Ciena Healthcare is seeking an experienced nurse to join our care management team as an MDS Nurse. You will handle the RAI process, conduct resident assessments, and coordinate care plans for assigned residents, ensuring timely MDS/CAA completion and PPS readiness.
You will work with physicians, nursing, social services, therapy, dietary and activity staff to identify needs, monitor Medicare days, and maintain PPS boards. RN or LPN with RAC-CT or AANC certification is a plus.
