Delivery of patient care.
- Provide personal care (ADL) including:
- Bathing (assist, bed bath, tub bath, shower, sitz)
- Body Mechanics (turn, position, transfer, lifts, ambulation, ROM exercises)
- Nutrition (preparing meals, feeding, measuring intake/output, snacks)
- Elimination (bedpan/urinal, bowel/bladder retraining, specimen collection, perineal/catheter care, condom catheter, douches, enemas, rectal tubes, drainage, gastric suction)
- Safety (side rails, call rails, mitts, restraints, CPR/Heimlich, infection control, isolation, universal precautions)
- Special procedures: vital signs, temperature, pressure, pulse, respirations, BP, height/weight, heat/cold, decubitus ulcer prevention, surgical skin preps, dressing changes, ace bandages, TEDs, binders, postmortem care, cough/deep breathing
- Housekeeping services for patient/family safety
- Participate in interdisciplinary team conferences (IDT) and special case conferences.
- As NA 1+ 4, provide additional:
- Sterile dressing for wounds over 48 hours old and wound irrigation
- Ostomy care or irrigation
- Insertion of urinary catheter limited to replacement of existing after nursing assessment
- Oxygen therapy (room set-up, monitoring flow)
- As NA II, provide additional:
- Breaking up and removal of fecal impaction
- I.V. fluid-assisted activities (assemble/flush, monitoring, dressing change, discontinuation)
- Nutritional activities (oral/nasogastric infusions, gastrostomy feeding, clamping, removal)
- Suctioning (oropharyngeal, nasopharyngeal)
- Tracheostomy care
- Insertion of urinary catheter same restrictions
- Sterile dressing for wounds and irrigation
- Ostomy care or irrigation
- Oxygen therapy
- Attend and participate in IDT and special care conferences.
- Maintain effective working relationships with team members.
- Identify patient/caregiver needs and refer appropriately.
- Ensure accurate, complete, succinct, and timely clinical documentation per HPCCR and licensure standards.
- Professional and Personal Accountability:
- Demonstrate competency, maintain professional reputation, and uphold professional development.
- Maintain licensure, CPR certification, CEU requirements.
- Build productive professional relationships.
- Exhibit time management and crisis response.
- Perform additional duties such as cleaning patient rooms, ensuring environment cleanliness, chemical use, protective equipment usage as required.
Minimum 1 year experience required. Experience in home care, working with terminally ill and/or geriatrics preferred.
