Home Health RN Case Manager & Recertification Nurse (RNCM)
Role OverviewThis is a Part-Time or PRN "Road Warrior" leadership position that requires travel to all client sites throughout Northwest Ohio. The RNCM works closely with and reports directly to the Northwest Ohio Regional Program Administrator.
Key Responsibilities- Perform client assessments while directing, planning, and initiating appropriate actions in home‑care situations.
- Manage and supervise a team of RNs, LPNs, Home Health Aides, and Personal Care Aides.
- Document, communicate, and coordinate patient status and changes in condition to the care team.
- Promote personal safety and a safe environment for clients and coworkers.
- Travel to surrounding counties as required.
- Provide back‑up and occasional skilled nursing visits, including medication sets and dressing changes.
- Case management, start of care (SOC), admissions, recertifications, transitional care, and resumption of care (ROC).
- Clinical reviews, wound care, PICC/PORT/IV care, laboratory draws, and OASIS/NON‑OASIS assessments.
- New hire orientations, in‑service training, supervisory reviews, and input on training, education, and care practices.
Road Warrior position covering the entire Northwest Ohio region:
Toledo, Holland, Perrysburg, Findlay, Richland, Marion, Mansfield, Fostoria, Tiffin, Norwalk, and surrounding areas. Availability is Monday through Friday, 8:00 AM – 5:00 PM, with occasional evening hours. Weekends are not required; weekend services are optional.
Long‑term employment for Part‑time or PRN RNCM/Recert Nurses. There are no on‑call or overnight travel requirements.
Compensation- Hourly rate $36 – $40 per hour, plus mileage reimbursement for round‑trip door‑to‑door travel.
- Overtime and quarterly bonus opportunities available.

- Graduate of an accredited School of Nursing.
- Minimum 1 year experience as a Registered Nurse with an active RN license in Ohio.
- Minimum 1 year of experience in a home health agency or equivalent setting.
- CPR and First Aid certifications required.
- Experience with OASIS and NON‑OASIS assessments.
- Strong background in nursing, patient care, confidentiality, and HIPAA regulations.
- Excellent documentation and field staff leadership skills.
- Consistent availability aligned to chosen schedule type (part‑time, full‑time, or hybrid).
- Physical ability to perform required duties (details provided during hiring).
- Background check and fingerprint clearance required.
- Valid driver’s license or , active U.S. work authorization, and reliable transportation within a minimum 2‑hour travel radius.
- Desirable traits: reliable, honest, professional, compassionate, detail‑oriented, accountable, friendly, respectful, and positive.
- Minimum 1 year of paid experience with a home health agency.
- Experience with nurse supervision, people management, and staff education is desirable.
- Successful relationships with client payors, case managers, and regional entities (e.g., AAA, DoDD, VA).
- Door‑to‑door mileage reimbursement.
