Job Title: Home Health RN Case Manager & Recertification Nurse (RNCM)

This is a Part‑Time or PRN "Road Warrior" leadership position with our agency, requiring travel as needed to all clients throughout Northwest Ohio. The role reports directly to the North West Ohio Regional Program Administrator and offers a defined path for career growth to higher leadership positions within the agency.

• Road Warrior position covering the entire Northwest Ohio region, including Toledo, Holland, Perrysburg, Findlay, Richland, Marion, Mansfield, Fostoria, Tiffin, Norwalk and surrounding areas.
• Willingness to travel throughout the region is required. A centralized base in Findlay/Fostoria/Tiffin is available, but applicants may reside anywhere within the service area as long as travel requirements are met.
• Weekday position (Monday – Friday) with primary daytime availability (8 am – 5 pm) and occasional evening availability. No weekend services are required, though weekend services are optional when available.
• No on‑call or overnight travel required.

Long‑term employment for Part‑time or PRN RNCM/Recertification Nurses. Recurring weekly schedule required. Short‑term or backup work only is covered under separate roles.

As an RN Recertification Nurse you will coordinate, re‑evaluate and confirm the medical necessity of continued home health care services for patients. You will provide direct patient care, ensure optimal outcomes, and oversee patient services with expert nursing care.

• Perform client assessments while directing, planning, and initiating appropriate actions in home care situations.
• Supervise a team of RNs, LPNs, Home Health Aides (HHAs), and Personal Care Aides.
• Document, communicate and coordinate patient status or change in condition to the care team.
• Perform job duties while promoting personal safety and a safe environment for clients and coworkers.
• Travel to surrounding counties as required; willingness to travel outside the immediate service area is expected.
• Minimum of one (1) year of nursing supervisory experience (home‑care experience preferred).
• Provide occasional skilled nursing visits and services, including medication sets and dressing changes.

• At least 1 year of experience in Case Management, Admissions & Discharges, wound care, PICC/PORT/IV Care, Transitional Care, lab draws, OASIS and non‑OASIS assessments, and excellent documentation and field staff leadership.
• At least 1 year of paid experience as a home‑health RN or in‑home care provider; 2–5 years preferred.
• Desirable experience:
  Nurse supervision, people management, peer/staff education and development, and building relationships with client payors and case‑management entities (AAAs, DoDD, County Board, VA, etc.) in Northwest Ohio.

• Graduate of an accredited School of Nursing.
• Minimum of 1 year experience as a Registered Nurse with an active RN license in Ohio.
• Minimum of 1 year experience in a home‑health agency or equivalent setting.
• CPR and First Aid certifications required (or obtainable prior to hire date).
• Experience with OASIS and non‑OASIS assessments is required.
• Strong background in nursing, patient care, confidentiality, and documentation.
• Consistent, on‑time attendance and availability aligned to chosen schedule type (PT / FT / Hybrid).
• Desirable traits: reliable, honest, professional, compassionate, detail‑oriented, accountable, friendly, respectful and positive.
• Familiarity with HIPAA regulations and medical terminology.
• Willingness and ability to perform required physical demands.
• Consent to a State & Federal background check (including fingerprints).
• 18 years or older, active Driver's License or State , US Work Authorization.
• Reliable transportation to all scheduled visits, within the full travel radius (at least 2 hours available).
