Job Title
Home Health RN Case Manager & Recertification Nurse (RNCM)
Job SummaryAs an RN Recertification Nurse, you will coordinate, re‑evaluate, and confirm the medical necessity of continued home health care services for patients, ensuring optimal outcomes and providing expert nursing care.
Key Responsibilities- Perform client assessments while directing, planning, and initiating appropriate actions in home care situations.
- Manage and supervise a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
- Document, communicate, and coordinate patient status and changes in condition with the care team.
- Perform job duties while promoting personal safety and a safe environment for clients and coworkers.
- Travel to surrounding counties as required.
- Require at least one year of nursing supervisory experience; home‑care experience preferred.
- Work includes case management, starts of care (SOC), admissions, recertifications, transitional care, resumptions of care (ROC), clinical reviews, new‑hire orientations, in‑service and supervisory reviews, and other assessments and services.
- Provide backup and occasional skilled nursing visits and services, including medication sets and dressing changes.
- Graduate of an accredited School of Nursing.
- Minimum of 1 year experience as a Registered Nurse, with an active RN license and ability to practice in the State of Ohio.
- Minimum of 1 year of experience in a home health agency or equivalent setting.
- At least 1 year of experience in case management, case admissions and discharges, wound care, PICC/PORT/IV care, transitional care, lab draws, OASIS and non‑OASIS assessments, and excellent documentation and field staff leadership.
- At least 1 year of paid experience in a home health agency or in‑home care provider; 2–5 years preferred.
- CPR and First‑Aid certifications required (or obtainable prior to hire).
- Experience with OASIS and non‑OASIS assessments required.
- Strong background in nursing, patient care, and confidentiality.
- Exceptional on‑time and attendance record, and consistent availability aligned to your chosen schedule (PT/FT/Hybrid).
- Desirable character traits: reliable, honest, professional, compassionate, detail‑oriented, accountable, friendly, accommodating, respectful, and positive.
- Familiarity with HIPAA regulations and medical terminology.
- Willingness and ability to perform required physical demands.
- Consent and ability to pass a State & Federal background check (including fingerprints).
- 18 + years old, active Driver’s License or State , US work authorization.
- Reliable transportation to all scheduled visits, within at least a 2‑hour travel radius.
- Desirable but not required: nurse supervision experience, people management, peer/staff education and development, success in building relationships with client payors and case‑management entities.
- Road‑Warrior position spanning the entire Northwest Ohio region, including Toledo, Holland, Perrysburg, Findlay, Richland, Marion, Mansfield, Fostoria, Tiffin, Norwalk, and surrounding areas.
- Willingness to travel throughout this region is required.
- Findlay/Fostoria/Tiffin are a centralized “home base” with minimal travel; applicants may reside near any service area as long as travel requirements are fulfilled.
- This is a weekday position (Monday–Friday) requiring primarily daytime availability (8 am–5 pm) with occasional evening availability.
- No weekend services required.
- Weekend services are optional when available but are not required.
Long‑term part‑time or PRN RNCM position. No on‑call or overnight travel required. Weekend services are optional.
Compensation- Part‑time or PRN (+ mileage): $36–$40 per hour, based on minimum 2‑hour travel radius and predetermined availability of visits/recertifications per week. OT and quarterly bonus opportunities available.
- Paid weekly (Fridays) via direct deposit.
- Round‑trip / Door‑to‑Door mileage reimbursement.
