Home Health RN Case Manager & Recertification Nurse (RNCM)

This is a Part‑time or PRN "Road Warrior" leadership position that requires travel across Northwest Ohio to serve clients. The role reports directly to the Northwest Ohio Regional Program Administrator and offers a defined career path in agency leadership.

• Conduct client assessments, admission, re‑certification, and transitions of care (SOC, ROC) while documenting patient status and medical necessity.
• Lead and supervise a team of RNs, LPNs, Home Health Aides, and Personal Care Aides.
• Coordinate care plans, documentation, and communication with physicians, agency staff, service administrators, and clients/families.
• Provide back‑up skilled nursing services, including medication sets and dressing changes, as needed.
• Advance quality of care through in‑service training and supervisory reviews; influence training, education, and care practices.
• Travel as required across the entire Northwest Ohio region (Toledo, Holland, Perrysburg, Findlay, Richland, Marion, Mansfield, Fostoria, Tiffin, Norwalk, and surrounding areas).
• Maintain a minimum of one year of nursing supervisory experience; home‑health experience preferred.

• Graduate of an accredited school of nursing with an active RN license in Ohio.
• Minimum one year of experience in home‑health or equivalent setting with proficiency in OASIS and non‑OASIS assessments.
• At least one year of case‑management experience in admissions, discharges, wound care, PICC/PORT/IV care, and transitional care.
• CPR and First Aid certifications (or willingness to obtain prior to hire).
• Excellent documentation skills and field‑staff leadership.
• Reliable transportation that allows travel within a two‑hour radius of scheduled clients.
• Phone, background check, and U.S. work authorization (driver’s license and state ).

• Nurse supervision experience.
• People management and staff education.
• Ability to build relationships with payors and case‑management entities in Northwest Ohio.

Long‑term part‑time or PRN employment with no overnight travel or on‑call requirements. Weekends are optional when available. The position is weekday‑focused (Monday‑Friday, 8 am‑5 pm) with occasional evening availability.

Part‑time or PRN hourly rate: $36–$40 per hour, plus mileage reimbursement for round‑trip travel. Overtime, quarterly bonuses, and flexible scheduling benefits are available.
