VTHH Critical Care NPs
Job SummaryThe specialty nurse practitioner in the respective setting works in collaboration with a multidisciplinary health care team. The specialty NP is generally service/physician aligned, managing a distinct patient population. The NP may be required to perform some coordination of outpatient and inpatient care and facilitate discharges/outpatient follow up. The specialty NP has a strong foundation in evidence‑based practice and education and is responsible for ensuring adherence to clinical pathways/protocols and ensuring an environment of safe quality care.
The NP demonstrates an advanced level of medical and nursing knowledge, clinical/technical competence, sound clinical judgment, professional communication skills, timely and compliant documentation, and is responsible for professional development and competency validation.
- Assessment of Health Status:
Perform complete history and physical examination for acute and complex chronically ill patients, order and collect data using appropriate assessment techniques and diagnostic procedures. - First responder role:
Serve as first responder for both nursing and house staff in event of patient status changes, orders, and situations requiring procedural and resuscitative interventions. - Risk assessment:
Assess risks associated with care of the acute and complex chronically ill patient including medication side effects, immobility, impaired nutrition, fluid and electrolyte imbalance, immunocompetence, invasive interventions and diagnostic procedures. - Clinic coordination:
Coordinate daily clinic schedule with the attending physician team, present patient report, collaborate in plan of care, and follow assigned patient population. - Diagnosis:
Manage diagnostic tests through ordering, interpretation, performance, and supervision. Formulate differential diagnoses by priority. Diagnose complications and order appropriate interventions. - Plan of Care:
Identify expected outcomes from diagnosis, formulate and document a plan of care to address complex acute and chronic health care needs. Utilize evidence‑based practice guidelines or protocols in an individualized, dynamic plan of care applicable across the continuum. Implement and modify plan of care. Prescribe diagnostic strategies and therapeutic interventions both pharmacologic and non‑pharmacologic. - Advanced procedures:
May perform advanced procedures consistent with privileges and competency validation, including minor suturing, complex wound management, line and tube placements and removals. - Communication and collaboration:
Maintain ongoing communication and collaboration with interdisciplinary health care team. Facilitate communication with patient, family and staff to promote continuity of care across outpatient continuum. - Documentation:
Document/dictate key components of patient’s progress via daily progress note, transfer, discharge summary, and clinic notes where applicable. Ensure documentation is timely, meets critical care compliance standards and captures patient acuity. - Professional practice:
Demonstrate professional practice behaviors including precepting/mentoring, education and instruction of students, nursing staff, nurses, graduate and novice nurse practitioners. Seek opportunities for active engagement in research and analysis of…
