Maternity Care Case Manager​/Lactation Consultant

JOB SUMMARY AND SPECIFICATIONS JOB SUMMARY Serves as a leader of the healthcare team. The MC Case Manager plans, directs, and coordinates the nursing care of the maternal child patient via the nursing process and in collaboration with the healthcare team. Collaborates and consults with health care professionals to facilitate appropriate patient movement across the continuum. Coordinates pre-admission, discharge, and follow-up care coordination.

The Case Management portion of this role is a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual's health needs through communication and available resources to promote quality, cost-effective outcomes. The MCC Case Manager is expected to be flexible with time scheduling and be aware of Major Hospital’s policies and procedures as well as assist in providing evidence-based practices.

Patient care and education will be delivered in outpatient and inpatient settings. The Lactation Consultant is responsible for providing lactation management services to patients and family in collaboration with other healthcare team members. Directly reports to the Director of Maternal Child and collaborates with the leadership team. MINIMUM QUALIFICATIONS Professional & Technical Skills Experience gained through an approved School of Nursing plus an appropriate period of hospital orientation to the assigned patient care area and to personnel.

Education Graduate of an accredited School of Nursing. BSN or obtain within 6 years (current ASNs with MHP refer to SPP No. HR-18) License(s) or Certification(s) Must possess a current Indiana license as a Registered Nurse. Other Skills or Requirements Lactation counseling experience preferred. OB experience preferred.
