Responsibilities

The Licensed Practical Nurse (LPN) in Home Health is responsible for providing quality patient care and performing technical skilled care in the patient's home. Provides services within the scope of practice defined by state laws, in accordance with the plan of care, and in coordination with other healthcare team members. May not alter the plan of care.

Instructs the patient's family in care and maintaining a healthy environment for the patient. Observes, records, and reports the patient's response to treatment and any changes in condition to the supervising nurse or physician daily.

Education and Experience

License Requirements:

• Current LPN licensure in the state of practice
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
• CPR Certification required

Additional State Specific Requirements:

• CA:
  One year of prior professional nursing experience as an LPN
• LA:
  Current LPN licensure in LA without restrictions and at least one year of experience as an LPN
• Other states:
  No additional specific requirements
