Pride Health is hiring a Registered Nurse to support our client's Home Health program in Shelbyville, KY (including Shelby County and parts of Eastern Jefferson County). This is a 13-week contract assignment (possibilities of extension or conversion) and a great way to start working with a top-tier healthcare organization!
Location & Coverage Area:
Shelbyville, KY (including Shelby County and parts of Eastern Jefferson County)
Job Type: Local Contract
Duration: 13-week contract (possibilities of extension/conversion)
Pay rate: $50 - $56/hr. on W2 (negotiable - pay is based on experience/educational qualifications)
Schedule:
Monday to Friday from 8 am to 4:30 pm (32 hrs./wk. guaranteed)
On-Call/Weekend:
Yes, one weekend a month.
Mileage:
Reimbursement provided (from first patient to last patient, excluding commute from home)
Job Responsibilities:
- Perform Start of Care (SOC), recertification, resumption of care, and discharge visits.
- Conduct comprehensive patient assessments and develop individualized plans of care based on physician orders.
- Provide skilled nursing care, patient education, medication management, and disease management in the home setting.
- Monitor patient progress, evaluate treatment effectiveness, and update care plans as needed.
- Complete accurate and timely documentation using Homecare Homebase (HCHB) EMR.
- Complete and maintain OASIS documentation in compliance with Medicare and agency standards.
- Collaborate with physicians, therapists, case managers, and other interdisciplinary team members to coordinate patient care.
- Educate patients and caregivers on diagnoses, medications, treatments, and home safety.
- Ensure compliance with agency policies, regulatory requirements, and quality standards.
Qualifications:
- Active, unrestricted Registered Nurse (RN) license.
- Ability to work independently in a home health environment
- 1 year of home health experience.
- HCHB (Homecare Homebase) EMR experience.
- OASIS Certification highly preferred.
