Role
Licensed Practical Nurse (LPN) providing direct patient care in home settings.
Responsibilities- Deliver high-quality nursing services within scope of practice, document patient visits accurately, participate in care coordination, communicate effectively with patients, families, and healthcare team, and ensure infection control procedures are followed.
- Current and unrestricted LPN licensure, valid driver's license, vehicle access or public transportation.
- CPR certification, flexibility, multitasking, independence, strong communication, organization skills.
