Job DescriptionThe LPN MDS Coordinator gathers information, assesses needs, establishes reasonable goals, provides interventions and incorporates them within an organized, concise, functional care plan. They coordinate the completion of comprehensive assessments by the interdisciplinary team and include recommendations in the written care plan for each resident.
The Coordinator works with the care planning team to implement final plans, encourages residents and their responsible parties to participate in development and review, and ensures plans focus on assisting residents to reach the highest practicable level of well‑being.
They ensure all nursing personnel are aware of each resident’s care plan, use the plans in daily nursing services, review nurses’ notes, monitor residents to confirm adherence to plans and that their needs are met, assess and revise plans as required, and schedule and conduct weekly care plan meetings for all residents according to OBRA and state requirements.
They complete the MDS with utmost accuracy to secure the highest level of reimbursement for the facility, maximize reimbursement, and ensure Medicare compliance in compliance with current CMS Mega Rule guidelines. Point‑Click Care experience is a plus.
ResponsibilitiesConfidentially gather and record resident data, assess functional status, develop care objectives, coordinate interdisciplinary assessments, develop comprehensive care plans, review and monitor nursing documentation, facilitate weekly care plan meetings, ensure compliance with CMS guidelines, and maintain accurate MDS records.
Compensation and HoursHourly rate: $37–$48 based on experience.
Hours:
16 hours per week.
- Experience completing Minimum Data Sets (MDS) and resident care plans in the long‑term care sector.
- Current state LPN license and good standing as a nurse.
- Meet all applicable federal and state licensure requirements.
- Attention to detail, strong follow‑through skills and ability to prioritize multiple tasks.
- Ability to instruct others.
- Knowledgeable of general, rehabilitative and restorative nursing and medical practices, procedures, laws, regulations and guidelines.
