Family Nurse Practitioner
A well-established, not-for-profit community hospital is seeking a family nurse practitioner (FNP) to join its outpatient primary care practice in Southbury, Connecticut. Candidates with a strong background in primary care, chronic disease management, and preventive medicine will be considered.
This is a full-time, permanent outpatient-focused position providing comprehensive care to a diverse patient population across the lifespan, with the opportunity to manage a wide range of conditions including diabetes, hypertension, asthma, and other chronic and acute illnesses. Both experienced nurse practitioners and new graduates are encouraged to apply.

Essential Responsibilities
- Provide comprehensive primary care services, including evaluation, diagnosis, and treatment
- Conduct routine physical exams, preventive screenings, and health assessments
- Manage acute and chronic conditions such as diabetes, hypertension, asthma, and infections
- Order and interpret diagnostic tests including labs and imaging
- Prescribe medications and develop patient-centered treatment plans
- Educate patients on disease prevention, lifestyle changes, and overall wellness
- Collaborate with physicians and multidisciplinary teams for coordinated care
- Maintain accurate and timely EMR documentation
- Participate in quality improvement and patient care initiatives
Qualifications
- Education:
Master's or Doctorate degree in Nursing from an accredited program - Certification:
Board Certified as a Family Nurse Practitioner - Licensure:
Active Connecticut APRN license (or eligibility) - Experience:
Open to both experienced providers and new graduates - Strong communication and clinical decision-making skills
