Family Nurse Practitioner
A well-established, not-for-profit community hospital is seeking a family nurse practitioner (FNP) to join its outpatient primary care practice in Southbury, Connecticut. Candidates with a strong background in primary care, chronic disease management, and preventive medicine will be considered.
This is a full-time, permanent outpatient-focused position providing comprehensive care to a diverse patient population across the lifespan, with the opportunity to manage a wide range of conditions including diabetes, hypertension, asthma, and other chronic and acute illnesses. Both experienced nurse practitioners and new graduates are encouraged to apply.

Qualifications include a master's or doctorate degree in nursing, board certification as a family nurse practitioner, active Connecticut APRN license, experience open to both providers and graduates, strong communication and clinical decision-making skills.
