Job Summary:
Gardner Heights Health Care Center, a 110-bed skilled nursing community located in Shelton, Connecticut, is seeking a compassionate and skilled Float Social Worker to join our dynamic team of three Shelton skilled nursing communities (Gardner Heights, Shelton Lakes and Hewitt). As a Float Social Worker, you will play a vital role in implementing and documenting plans of care related to residents' social/emotional needs, collaborating with multidisciplinary teams, and ensuring excellent resident care outcomes.
This role is in addition to the three full time Social Workers stationed at each location. This is a full time benefit eligible position scheduled at 32 hours per week.
Implement and document plans of care related to residents' social/emotional needs in the electronic medical record
Collaborate with nursing, MDS, and rehabilitation departments to facilitate discharge plans for residents
Inform and educate staff, residents, families, and responsible parties of residents' personal and property rights, as well as their rights related to health services
Maintain updated resources for residents and families regarding community resources and services
Investigate reported concerns, assist in resolving them, and ensure appropriate follow-up and documentation
Provide timely referrals to appropriate agencies for assistance to residents and families
Bachelor's or Master's Degree from an accredited program in Social Work or related field
Previous experience in a long-term care facility, preferably with knowledge of current Requirements of Participation regulations
Knowledge of admissions and discharge procedures, as well as residents' rights and documentation requirements
Attention to detail, good follow-through skills, and ability to prioritize multiple tasks
Innovative and able to find creative ways to serve the variety of resident needs
Must have a vehicle and a valid driver's license
Demonstrates cooperation in assisting others and ability to work as a team
Patience, tact, enthusiasm, and a positive attitude toward residents and their support networks
Ability to remain calm in stressful situations
Ability to communicate with residents, staff, and family members in a professional manner
