This 1.0 FTE/Full-time position is scheduled to work 8-HR day shifts.

Compensation:
Exempt, $39.48/HR -$56.40/HR. Placement within this range is based on years of applicable experience.

Additional Compensation:

Job Summary:

As an Emergency Department Social Worker at Mason General Hospital, you will provide essential psychosocial support to patients and their families in a fast-paced, high-stress environment. Your role will involve assessing and addressing the emotional, social, and practical needs of individuals in the emergency department, facilitating crisis intervention, and coordinating with multidisciplinary teams to ensure comprehensive patient care.

Job Specific Duties & Responsibilities

Patient and Family Support:

Conduct psychosocial assessments to identify patient and family needs.

Provide crisis intervention, emotional support, and counseling to patients and their families.

Assist with emotional, social, and practical aspects of patient care, including coping with trauma, grief, and loss.

Resource Coordination:

Connect patients and families with community resources, support groups, and other social services.

Facilitate discharge planning, including arranging for follow-up care, referrals, and ensuring a smooth transition from the hospital to home or other care settings.

Assess and assist with Social Determinants of Health needs of the patient.

Multidisciplinary

Collaboration:

Work closely with emergency department physicians, nurses, and other healthcare professionals to develop and implement patient care plans.

Participate in case conferences and contribute to team discussions about patient care.

Crisis Management:

Address immediate safety concerns, such as potential for self-harm or harm to others.

Manage cases involving abuse, neglect, and other significant social issues.

Documentation and Compliance:

Maintain accurate and confidential records of patient interactions, assessments, and interventions.

Ensure compliance with hospital policies, legal requirements, and ethical standards.

Education and Advocacy:

Provide education to patients and families about their rights, available resources, and the healthcare system.

Advocate for patients' needs within the healthcare system and the community.

Required

Education and Experience:

Minimum of 2 years of social work experience. Healthcare or Emergency setting preferred.

Minimum of 3 years of experience in hospital or clinic coding.

Required Licenses, Certifications and/or Registrations:

Current state licensure as a Licensed Clinical Social Worker (LCSW) or equivalent.
