Case Management offered at Area Agency on Aging Mason County
2008 Olympic Highway North, Shelton, WA 98584
Adults age 60 and older, and/or disabled individuals who meet a needs-based and financial eligibility assessment (performed by DSHS Home & Community Services office) in Lewis, Mason and Thurston Counties.
Case Managers assess the temporary and permanent needs of older and disabled adults who are receiving Medicaid in-home personal care services to coordinate these services in order to maintain maximum independence.
Case Manager responsibilities include:
- Assess needs of individuals on State-funded Medicaid personal care services.
- Develop care plans.
- Authorize and arrange services.
- Coordinate provision of services among providers.
- Follow-up with re-assessments as needed.
Lewis-Mason-Thurston Area Agency on Aging plans, coordinates and advocates for services for people ages 60 and older, disabled adults and unpaid family caregivers.
