Travel RN - Home Health

** Shepherdsville, KY 40165

* Schedule:

** Monday - Friday |  
** 8:00 AM - 4:30 PM**  |  
** 32 hours/week guaranteed*
* ** On-Call:
** One weekend per month

** Coverage Areas:
** Shepherdsville, Bullitt County, and Jefferson County, KY

** Mileage:
** Reimbursed between patient visits, from the first patient to the last patient.

** Responsibilities of the Travel RN:*
* · Provide skilled nursing services in patients' homes in accordance with physician orders

· Complete Start of Care (SOC) admissions, comprehensive assessments, and ongoing patient visits

· Coordinate care with the interdisciplinary healthcare team

· Maintain accurate and timely clinical documentation

· Deliver high-quality, compliant home health patient care while promoting positive patient outcomes

** Qualifications for the Travel RN:*
* · Active Registered Nurse (RN) license

· Home Health nursing experience required

· Strong assessment, documentation, and patient care skills

· Excellent communication and organizational abilities

· Ability to work independently in a home health setting

* Preferred Qualifications:

*
* · Experience with Homecare Homebase (HCHB) is a plus

· OASIS documentation experience preferred
